To ensure accurate billing, you will be asked for current insurance information each time you check in for an appointment at the office. You will be asked to review all your personal information upon check in. Please review this information carefully and alert our staff of any changes.

 

All copays are due at the time of your appointment.

 

We accept all major credit cards, personal checks and cash. You are responsible for paying any deductible, co-insurance, or other balance that is not paid by your insurance company. Accounts are due upon receipt of your first statement.
